Steps

Becoming a patient

1. Schedule your evaluation. Call our office at 972-332-5922 or request an appointment online. We do not require a referral, although some insurance plans do, and prior records are helpful if you have them.

2. Share your insurance information and complete your intake forms. When you request an appointment, we ask for your insurance details first so we can confirm your benefits before you are seen. Once your visit is scheduled, the full intake forms are sent through the patient portal. Please complete them before your visit. They cover your history, your current symptoms, and your medications, and completing them in advance allows more of the appointment to be spent on your care.

3. Attend your evaluation. The initial psychiatric evaluation is 60 minutes and takes place in our office. Please bring a photo ID, a list of your current medications or the bottles themselves, and any prior records you have, including previous evaluations, testing, and laboratory results. You are welcome to bring a family member or another person with you.

4. Review the plan and schedule follow-up. During your visit, we will determine a working diagnosis and develop a care plan that is right for you. We will discuss the treatment options, what each involves, and how often you should be seen, and a follow-up appointment is scheduled.

Who we see

We see patients 16 and older throughout the Dallas–Fort Worth area. Initial evaluations take place in our Hurst or Dallas office, and follow-up appointments may be conducted in the office or by video, with telehealth available to patients located anywhere in Texas.

There are situations in which we are not the right practice, and we will tell you when that is the case and help you find a practice that is. We do not provide crisis services or residential detoxification services. We do not perform court-ordered, forensic, or custody evaluations.

First visit

Your first visit

The first visit is a psychiatric evaluation. We will review your history, current symptoms, prior treatment, medical conditions, and goals of treatment, and we will discuss a diagnosis and a treatment plan with you.

Whether a medication is prescribed at the first visit depends on what the evaluation finds. Where the evaluation supports starting medication, a prescription may be provided at that appointment. Where more information is needed — records from a prior clinician, laboratory testing, collateral information from a family member, or a second appointment to complete the assessment — we will explain what is needed and how we will proceed. Certain evaluations, including the evaluation of ADHD, commonly require more than one appointment.

New patient appointments are for evaluation and treatment, not for FMLA, disability, or workers' compensation paperwork. Forms and letters are completed for established patients, during a scheduled visit, once we have been working together about three months. Follow-up

After the first visit

Follow-up appointments are scheduled as either a 25-minute medication follow-up or a 50-minute medication-plus-therapy follow-up, depending on your treatment plan. Appointments are usually more frequent early in treatment and after a change in medication, and may be spaced further apart as symptoms stabilize. Patients receiving prescriptions are seen at least every three months, and more often when the medication or the diagnosis calls for it.

Between appointments, you may reach us through the patient portal or by calling the office. Refill requests go through the portal or your pharmacy.

If your treatment plan includes TMS or Spravato, our office will handle the prior authorization process and will keep you informed along the way, or provide a written quote if you are paying out of pocket.

New patients begin with a 60-minute evaluation at our Hurst or Dallas office. We are in network with most Blue Cross and Blue Shield of Texas plans, Aetna, Ambetter from Superior HealthPlan, and Medicare. Self-pay rates are listed on the Insurance and Fees page.
